Becoming a Patient
First-time patients (with the exception of emergency-care patients and those referred to our specialty clinics) must be evaluated. If you are interested in becoming a patient at the school, contact our call center at (213) 740-2800 to schedule an initial appointment. At this appointment it will be determined if you meet our education criteria to become a patient at the school.
Once you are evaluated, you will be assigned to a student within a particular group practice, overseen by one of our faculty. The group practice director is responsible for overseeing the treatment plan for each patient.
The fee for the initial visit, including basic x-ray is $95. For some patients there may be a fee for additional x-rays.
You also may become a patient of one of the school’s faculty practice. The cost is comparable to the going rates of private dental practices and approximately three times the cost of care provided in our student clinics.
Incoming Patient Paperwork
We encourage prospective patients to print and fill out incoming patient paperwork to speed up the check-in process.
All patients must bring a list of all their medications, with dosages information, at their first visit. This includes any prescriptions, over the counter medications as well as any herbal or vitamin supplements.
Some patients may have medical conditions that require a consult with your medical provider prior to dental treatment being rendered for you. Please bring in your current medical provider’s contact information with you on your first visit.
If you have a medical condition that requires you have laboratory blood test to monitor your condition or medications, please bring in your current lab values. For example, if you are diabetic, please have your current hemoglobin A1c (HgbA1c) or if you are on an anticoagulant medication that requires INR blood test monitoring – please bring in your current result.
Please note that bringing filled-out paperwork does not guarantee prospective patients access to a dental professional at the Herman Ostrow School of Dentistry of USC.
